How about the Gravity forms Add-On called Signature ?

If you are already using Gravity Forms, using an officially sanctioned add-on like Signature would be a good choice. I’ve no experience with Gravity Forms nor Signature, so I cannot comment further on suitability.

I do know that Gravity Forms is a well liked and frequently suggested form builder plugin.
